        My role as a social worker/patient advocate is to guarantee that the residents have a voice, feel as if they have someone protecting their rights, and know that someone has their best interests in mind. In my position at I.O.O.F., I meet with all residents and families upon admission into the facility in an attempt to acquaint myself with each and every family.
        I also assess residents quarterly so that I am certain their needs are being adequately met, and I am involved in all care plan meetings to make families aware of their loved ones’ level of social functioning. I also refer patients for services they may need, such as health services and/or mental health services. As a licensed social worker, I counsel residents individually when the need arises and try to remain aware of all declines and/or improvements in the mood of residents. I make every effort to ascertain symptoms of mental health problems prior to them becoming an obstacle in the residents’ care.
        Finally, as the I.O.O.F. social worker, I follow the residents’ progress so that their discharge plans can be correctly developed. During discharge planning, I make certain that residents are connected with needed services, such as home health and/or equipment. I also work directly with Medicaid and Medicare so that residents receive their benefits appropriately.
