DEPARTMENT HEADS

Administrator:

      Kelly Wolfe, L.N.F.A. worked for eleven years in long-term care before becoming the facility’s Administrator in 2003. He attended Stephen F. Austin where and received his Bachelor Degree in Social Work. He then worked in adoption and foster care for Child Protective Services in East Texas. Mr. Wolfe received his Post-Baccalaureate Degree in Health Care Administration. In 1994 he made the decision to devote his career to long term care. He loves spending time with his family and fishing. He is especially proud to be a part of such a great team and organization who truly holds the interest of residents in their hearts.

Director of Nursing:

   Nancy Ivie, R.N. received her R.N. certification from Navarro College in 1989.  Since that time, she has traveled throughout the state of Texas working in many facets of nursing, including hospital care, home health care/primary home care, hospice care, nursing home care, and contract work at various health facilities.  Her major responsibilities as the Director of Nursing is to oversee the quality of patient care and ensure that patient's individual plan of care is being followed.  She is responsible for total resident care and supervision of nursing activities.

Assistant Director of Nursing:

      Paula Casmer, L.V.N. received her L.V.N. Certification from Johnson Memorial Hospital for Vocational Nurses in 1963. Her major responsibility as Assistant Director of Nursing is the staffing of nurses and C.N.A.s. Paula has been with the IOOF for the last 10 years in this capacity. She has 28 years experience working in hospitals. Her passions are reading and spending time with her grandchildren. Two of her daughters are R.N.s.

Medicare-Medicaid Coordinator & Care Plan Coordinator:


      Donna Lemire, L.V.N. received her L.V.N. Certification from Navarro College and is pursuing her R.N. and B.S.N. certification. Donna owned and managed her own restaurant for 10 years and spent 17 years as Training Supervisor for Pepsi Cola. As Medicare-Medicaid Coordinator she gathers data for the state to ensure revenue for the nursing home. Donna meets personally with each resident and family to discuss the resident’s individual plan of care. Her leisure time is spent gardening, reading, traveling, and spending time with her family.

Admissions Coordinator:
       Jeanne Englehart, L.V.N. has been the facility’s Admissions Coordinator since December 2004. Jeanne received her L.V.N. Certification from Navarro College. She worked in the Postpartum Unit at Charlton Methodist Hospital for 6 months. Jeanne worked P.R.N. at IOOF before becoming Weekend Charge Nurse working two double shifts on weekends for 3 years. As Admissions Coordinator her responsibilities include family tours of facility, completing all admission paperwork, evaluation of new residents, physicians’ orders, and M.A.R.s. Her ultimate goal is to receive her R.N. degree. In her leisure time she enjoys spending as much time as possible with her children. Her e-mail address is jeanne_englehart@hotmail.com.

Patricia Boyde

In-Service Coordinator & Nurse Aid Instructor:


     Melissa Caldwell, L.V.N. received her L.V.N. certification in August 2001 from Navarro College. She has been a Charge Nurse for this facility since November 2001. Melissa was promoted to In-Service Coordinator & Nurse Aid Instructor in September 2005. Her responsibilities include providing in-service training to meet state requirements, training and orientation for C.N.A.s, new employee orientation, dietary; and pharmacy recommendations, and providing flu shots and TB testing to residents and employees.

Activity Director:

     Kay Cobb, A.D. received her A.D. certification from McClennan Community College in 1999.  Since that time, she has worked at three different nursing homes in Corsicana, TX and worked at a nursing home in Rowlett, TX.  Prior to becoming an Activity Director, she was a pharmacy technician and a dental assistant.  She has always had a passion for helping others and enjoys ensuring that the nursing home residents live a joyful, fun-filled life.  She enjoys collecting and listening to music, she loves to read, and she loves to spend time with her four grandchildren.

Gena Percival, CNA

Environmental Services Supervisor:


      Dean Roberts has been the facility’s Environmental Services Supervisor for the last 11 months and has 20 years experience in this capacity. Dean worked in various departments at Children’s Hospital in Colorado for 14 years. He is a graduate of Broadcasting School, has his FCC license, and is a former D.J. He has been coaching little league for 4 years, and was the IOOF Hornets coach this year. Dean loves working with children and the elderly.

Director of Rehab:


      Tara St. Clair is the Director of Rehab Facility Operations and has worked in this facility for 6 years. She is a Certified Occupational Therapy Assistant with significant experience in the areas of long-term care, pediatrics, and acute care. Tara also has 7 years of overall experience in adult and geriatric rehab. He goal is for every patient to reach their maximum functional ability. She enjoys fishing and camping and spending time with her family, especially her soon-to-be 3 year old.

Social Worker:


       Jessica Henson, L.B.S.W. graduated from University of North Texas in December 2002 where she received her Bachelor's degree in Social Work and is currently working toward her Master's Degree in Social Work at UT Arlington. She attended Navarro College her freshman year on a fast pitch softball scholarship. Jessica worked as an investigator for Child Protective Services for two years before becoming the IOOF Social Worker. She is the liaison between families and the nursing home and works individually with each resident to complete assessments, counsels residents, and is very involved in each resident’s care plan. In her leisure time she enjoys art, reading, fishing, sports, and spending time with her family.

Maintenance Supervisor:

       Neil Markham has worked in healthcare maintenance for over 18 years.  In his many years of experience, he has worked at healthcare facilities in Dallas and worked at Baylor Healthcare in Waxahachie and Ennis, TX.  He is recognized as a mastered electrician in the state of Texas.  Neil lives on a farm, which requires much work but in his spare time enjoys fishing and spending time with his family, including six grandchildren.
